I hereby release the ISIJ of Toronto, its executive committee, its members, volunteers and organizers of the spiritual retreat (the "Releasees") from all claims and damages including personal injury, death, property loss or damage, and further waive any liability in negligence against the Releasees and voluntarily assume any foreseeable and unforeseeable risks associated with the spiritual retreat. I will conduct myself in an appropriate manner as it pertains to my own limitations and not recklessly or negligently assume any risks that are beyond my own capacity to bear.
I am aware that the physical activities, included in the retreat, involve risk and that the weather may change rapidly and may be extreme, presenting significant challenges and that if I choose to participate in any such activities, then I will be doing so at my own risk.
I further certify that I am medically, physically, emotionally and in all respects fit and able to participate in the retreat. I have no medical requirement or condition except what is outlined in the Registration/Medical form.
